Job Description
PurposeTo provide assistance to the resident in a manner that meets or exceeds facility expectations.
Primary Job Responsibilities
General
- Provide stand-by assist (or reminders) with activities of daily living such as grooming; oral hygiene; bath/shower; hair; maintaining the personal environment (i.e., clothing, linen, personal belongings with the apartment).
- Provide information and encourage residents to participate in community activity program.
- Transport residents to other departments and/or functions.
- Respond to emergency calls, resident/visitors requests and correct environmental hazards in an appropriate and safe manner.
- Document/report care provided and resident observations including unusual or significant changes in physical, mental, or behavioral conditions.
- Obtain/perform vital signs and heights and weights.
- Know and is able to respond to emergency needs such as designated by community policies.
Essential Functions
- Must be able to communicate and function in an interdisciplinary team.
- Must be able to sit, stand, bend, lift, push, pull, stoop, walk, and move frequently during working hours.
- Must be able to lift 40 lbs. floor to waist, lift 10 lbs. waist to shoulder, lift and carry 40 lbs., and push/pull 40 lbs.
- Must be able to read, write, speak, and understand the English language.
- Must be able to assist in the evacuation of residents.
Job Requirements

- Accept other duties as assigned.
Education
- Current LPN License.
- CPR certification preferred.
- Current state registration.
- Three- to five-years experience in assessment and geriatric nursing.
- Graduate from an accredited school of practical nursing.
