Recreation Coordinator Position Summary: Under the supervision of the Center Manager, and as a member of the Senior CommUnity Care Program the Recreation Coordinator assesses the needs, interests and capabilities of participants and develops individualized therapeutic recreation plans. In addition, plans, organizes, directs and participates in a comprehensive therapeutic recreation program tailored to the general and individualized needs and limitations of frail elderly program participants. Provides leadership and support to Recreation Assistant.

Essential Functions of the Recreation Coordinator:
1. Conducts an in-person initial comprehensive recreational assessment. Coordinates with the Interdisciplinary Team to develop a comprehensive plan of care for each participant.
2. Conducts an in-person reassessment semi-annually and as needed.
3. Functions as a member of the Interdisciplinary Team. Maintains regular attendance at, and participants in, Interdisciplinary Team meetings; communicates participant changes, collaborates on plan of care decisions and coordination for twenty-four (24) hour care delivery.
4. Plans and implements group and individual activities to improve or maintain the participants cognitive, physical, social, psychosocial, and artistic needs.
5. Plans and implements activities for dementia-specific population. Oversees daily flow of dementia activities area. Activities may focus on small groups, one-on-one programming and sensory stimulation.
6. Organizes community outings and special events at the Adult Day Health Center (ADHC). Duties include: arranging for participant transportation, entertainment, catering and involving other staff.
7. Develops a monthly calendar of activities.
8. Evaluates program plans to promote effective programs according to client abilities and preferences within the ADHC daily flow of events.
9. Provides recreational educational and training to support Activity Assistants, and other PACE Center staff members, as appropriate.
10. Provides on-going support and training to all staff involved in dementia-specific programming related to the dementia disease process.
11. Maintains participants medical record and fulfills Senior CommUnity Care charting and reporting requirements.
12. Provides leisure education to participants and caregivers for activity modification, use of adaptive equipment, activity skill development, and recommended supplies for in-home use.
13. Acts within the scope of his or her authority to practice
14. Maintains general order in the activities area, including daily clean-up, routine supply inventory ordering, petty cash and overall general budgeting.
15. Works cooperatively with other staff and departments to meet Senior CommUnity Care participants needs.
16. Assists participants with non-skilled Activities of Daily Living (ADL) as needed.
17. Protects privacy and maintains confidentiality of all company procedures, results and information about employees, participants and families.
18. Participates in and supports Quality Improvement Initiatives.
19. Performs related duties as required.

Job Requirements

Qualifications for the Recreation Coordinator:
1. Must have medical clearance for communicable diseases and up-to-date immunizations before having direct participant contact.
2. Must have a valid drivers license, proof of insurance and have means of transportation.
3. Education: Bachelors Degree in Recreation Therapy or related field preferred. Education or training in geriatrics, including dementia, preferred. Be legally authorized, currently licensed, registered or certified if applicable to practice in the State of Colorado.
4. Experience: Must have two (2) years experience in a social or recreational program providing and coordinating services for a frail or elderly population. Must have proven experience working with seniors.
5. Skills and Knowledge:
Working knowledge of recreational therapy techniques and practices of the elderly including assessment, care plan development, monitoring and evaluation needs.
Working knowledge of physical, psychosocial and behavioral needs of the elderly population.
Excellent oral and written communication skills.
Demonstrated creative abilities to plan, develop and implement varied activities to meet diversified needs.
Ability and experience working with volunteers.
Good working knowledge of dementia disease process.
Familiarity with illnesses common to frail older adult population.
Ability to utilize computers and other electronic devices for tasks such as timekeeping, in-servicing and documentation.
Knowledge of documentation and assessment process.
Recreation programming and goal driven/focused activity planning for all ages.
