REGISTERED NURSE - RN

Life Care at Home in Alamosa, Colorado

PRN position available. (EOE/M/F/V/D)

REQUIREMENTS

The Registered Nurse is responsible for the clinical care of assigned home care patients which may include but not be limited to: case management, specific nursing tasks, patient assessment, patient/family education, clinical and progress note preparation, in-service participation, and coordination with the multidisciplinary team.

• Knowledgeably and competently delivers nursing care to patients


• Follow physician orders and nursing treatment plans for patients


• Evaluates patient needs through ongoing assessment and revises care plans based on changes in patient condition


• Oversees and evaluates care given by LPN and HHA


• Identifies medical supply needs of patient and ensures patient has adequate supplies to meet care needs


• Contacts physician and appropriately requests other disciplines be assigned to case; coordinates the development of plans of care with attending homecare physician


• Regularly re-evaluates patients to identify progress towards goal achievement. Reviews OASIS answers, coding and sequencing of diagnoses to ensure accuracy; obtains orders and updates Plans of Care as needed throughout the episode of care


• Documents and charts appropriately and within specific time frames


• Participates as part of an interdisciplinary team to assure that patient services are delivered in a timely and effective manner

Job Requirements

EDUCATION and EXPERIENCE

• Licensed as a Registered Nurse by the state in which practicing


• Minimum of 1 year experience as a RN


• Graduate of an approved school of professional nursing


• Home Health experience preferred


• CPR certification

SPECIFIC REQUIREMENTS

• Valid drivers license and auto liability insurance


• Reliable transportation to travel to patient homes per patient schedule and agency needs


• Knowledgeable of nursing practices and procedures based on state practice acts


• Ability to make independent clinical decisions


• Able to (or assist with) lift, turn, move and position patients
