Job Description

Arvada Care & Rehabis a busy skilled nursing facility with a fast paced in-house therapy program. Conveniently located in the Denver Metro area, we are proud of our facility and grounds as well as our excellent facility team. We are running both inpatient and outpatient programs and have dynamic therapy programs and an excellent facility staff. We currently seek a per diem PT to join our fun and hard working therapy department. Hours will include occasional weekends. As part of our team you will have exposure to our existing programs in neurology, ergonomics, orthopedics, splinting, manual therapy, and many more. Arvada Care & Rehabilitation Center receives support from Ensign Services who provides a wide range of clinical and operational assistance to over 100 independently owned healthcare operations located throughout the United States. Duties:

• Screen and/or evaluate patients
• Develop effective treatment plans and obtain approval for services from referring physician
• Treat patients according to the physician treatment plan
• Supervise Physical Therapy Assistants and/or Physical Therapy Aides in direct patient care and patient related activities, following state practice act and adherence to facility Supervision Policy and Procedure Matrix
• Communicate with supervisor and other health team members regarding patient progress, problems and plans
• Participate in Patient Care Conferences, Utilization Review meetings and Rehabilitation Conferences
• Participate in in service training program for other staff in the facility
• Record evaluations, daily treatment notes, weekly progress notes, HCFA 700 & 701, per the facilitys policy and procedure
• Record treatment changes daily per policy and procedure
• Instruct patients families or nursing staff in maintenance programs
• Participate in discharge planning
• Coordinates necessary durable medical equipment for patients to facilitate safety and independence in mobility; i.e., braces, walkers, canes, elevated toilet seats and grab bars
• Report any problems with department equipment so that it is maintained in good working order
• Act as a clinical preceptor for affiliating Physical Therapy or Physical Therapy Assistant Schools
• Comply with the Physical Therapy Practice Act in the state and the facilitys policy and procedures

Experience/Qualifications:Licensed or Certified Physical TherapistNew graduates are welcome
